Full, well-contoured faces are attractive. We’re born with nourishing and hydrating facial collagen and supporting tissues. As we age, those structures can decline, often resulting in a “sunken or hollowed out” appearance. Dermal fillers are natural (fat) or synthetic injectable materials that restore natural and youthful-looking facial volume, smoothness, and contours. These fillers can restore that volume and help maintain your skin’s healthy, youthful appearance. You may also hear dermal fillers called soft tissue fillers.
Fat transfer dermal filler injections are surgical treatments that produce excellent results for both the skin and facial contours, requiring minimally invasive surgical procedures to acquire the filler substrate. The preferred candidates for dermal filler injections are in good general health. Tell your doctor if you have a bleeding disorder or chronic allergies. You will be evaluated for lidocaine allergy since that numbing agent is often used in filler injections. Filler injections cannot be performed if you have an active infection.

We then perform a complete ultrasound of your face in order to keep you safe during the injections so as to be able to avoid the risk of accidental injection of filler into an artery. Your injections will be performed in an office setting. After numbing your skin with topical anesthetic cream or nerve blocks, your provider will perform a complete ultrasound of your face highlighting your arterial anatomy. Once we know your particular anatomy, we use a series of small injections to place filler material into your treatment areas. This doesn’t take long–typically 20 minutes or so, depending on the treatment area. After that, you can immediately resume most normal activities.
The plumping and shaping produced by your filler injections will be apparent right away, but it may take about two weeks for the filler materials to settle into their final positions, revealing your full results.
It’s a good idea to avoid strenuous activity for a day or so. Do not apply ice to your injection sites if your face has had local anesthesia injected into it. The anesthetic contains epinephrine, which constricts blood vessels and disables their ability to react normally to cold stress, which may cause frostbite. In order to eliminate or reduce post-injection soreness, Motrin or Tylenol, as directed, is a great help. If you are already in the habit of using high-quality sunscreen daily—good for you! If not, now is an excellent time to adopt that routine to maximize your results. In many cases, dermal filler results can last longer than a year; in some cases, they still present several years after injection, depending on the filler and location.
Ultrasound scans of the face performed before injecting filler is the number one way to prevent accidental arterial placement of filler. The Aesthetic Plan has decided to be one of the few facilities in the area that uses this diagnostic tool to safeguard its dermal filler and fat transfer patients. Filler, when injected into an artery, can cause an occlusion of blood flow in that vessel, which then causes the tissues fed by that artery to die. Ultrasounds guide the injector to areas that do not have arteries in the path of the injection and thereby avoid the catastrophic effect of accidentally blocking off an artery and suffering tissue death as a consequence (e.g. blindness, etc.). In cases where an artery was injected with filler, the ultrasound also assists the provider in pinpointing the exact location of the occluded artery, enabling injection of the vessel with Vitrase (a filler dissolving solution), which dissolves the filler and returns blood flow to the artery and the tissues that it feeds. The patient will have no injury and no devastating sequelae as a result!
Some people need a follow-up treatment in six months, but others maintain full results for years. And here’s an added advantage: Dermal fillers are reversible if you change your mind.
Dermal fillers do not provide the dramatic differences often produced by a facelift, brow lift, or eye lift; however, fat transfer fillers can be used to provide more lifelike natural contours postoperatively after those surgical procedures.
